How big is your practice? 20-30 GPs? If you have 3-4, I'd just use MD3 or BP on an XP Pro, or even 2000 Pro, box. The only time I'd look at an MS server OS is if there is any prospect of reaching the 10 PC connection limit that MS imposes on their non-server OS's.

As for MSDE, I'm sure with even 8-10 GPs all at it at once it would work OK. I've yet to hear a report of a practice going with MSDE, then moving to full MS SQL server.

I'm assuming you don't need another DHCP server, a domain, active directory and all the other enterprise-level stuff that 99% of practices with an MS server OS will never use.

The '10 PCs and its a server OS' rule has always been a rip-off.
