No, I haven done that but the 530 and 560 snapgears which we have do allow packet filtering between the VPN interface (which I assume means both the ipsec and PPTP interfaces) and LAN interface so it just needs to be set to allow only the protocols and clients you require to have access to the VPN. If you are only allowing rdp through the tunnel to the lan at the surgery end and blocking access to the tunnel to all but a trusted computer at the home end I think you are pretty safe provided the usual antivirus anti-malware precautions are being used on the client machine.
