You don't.
And in the absence of a real privacy legislation with teeth, all you can is 
cross fingers and hope the banks behave ethically (who is it who just said 
someything about dreamers on this list?)

Look, if you want privacy, nothing beats cash transactions - keep it in mind 
and your doctor will appreciate it too. Simplest of transaction terms - hand 
over the dollars, we hand over the receipt, and we are both happy.
