This does not represent the full story.  Medicare or the government did
not just sit down and think of a number between from zero and infinity,
and announce that that would be the transaction fee.

I understand that the 23 cents per transaction has been determined by
negotiation between the government and the banks, and is a compromise
between what the banks would have liked to charge (presumably a lot
more) and what the government is willing to pay.  I understand that the
price of 23 cents was calculated by government as hopefully enough to
make the deal attractive to the banks while minimising cost to
government.  With only three banking organisations signed up to date, it
looks as if the 23 cents may not be enough.
