If we really want practices or groups of practices such as the Collaboratives program to act as incubators for innovative re-use of the collected data then (1) to (3) are a joke and should not even be considered. Unfettered live read-only access is the absolute minimum. I don't agree that this would be more burdensome for vendors - all they have to so is remove the artificial barricades between the customers and their data.
