I can write and delete files to the device with no problems, so it
doesn't look like a mount problem. When I press yes, it appears to
sync correctly (and possibly does change something - it loses my
'current location' in the podcast I'm listening to). But the episodes
don't appear on the device.

One thing I did find, the following files are present on the device:

com.apple.dbaccess.lock
com.apple.itdbprep.postprocess.lock
com.apple.itunes.lock_sync

Would these be causing any problems?

Sure, will do. The account creation worked - I guess I was just being
impatient. It took a couple of hours for the email to arrive. :)
