Hello,

the long-time asked feature of putting two lines of annotation above the 
initial finally did it to the latest svn version. Commit 798 adds the macros:

 * \setsecondannotation with only one argument, the text of the second line
 * \setaboveinitialseparation with a dimension as argument. This dimension is 
the separation space between the first and second line of annotation (default 
is 0.5mm)
 * \setfirstannotation with only one argument which is simply a wrapper for 
\setfirstlineaboveinitial{#1}{#1}.

Also, commit 799 adds additional lines for the custos of very high and low 
pitches. These lines have the same color as the other lines.
