Addendum:
Just realize that my MWE had used a personal package (localfuncs). This
package (re)defines several convenience commands for me. The only one
in use in this document is \th, which has been redefined to produce a
superscripted "th" rather than a thorn character. This does not effect
the problems I'm having either way so you can simply comment out/delete
the line "\usepackage{localfuncs}". Sorry about any confusion this
might cause.
