Growl doesn't check anything; in fact, Growl has nothing to do with Mail. GrowlMail doesn't check anything, either; it listens for the notifications Mail posts when it adds a message to a mailbox. You may be on the right track, though. IMAP servers, especially Gmail, sometimes do put the same message into multiple mailboxes (e.g., Gmail's “All Mail”). Mail models this as multiple messages, so GrowlMail has no way to tell that they are actually the same message.
