In GT4 it is possible if you use a host cert whose CN matches the result of a
reverse DNS query of the public facing host's IP address.

Or if clients don't use host-based authorization.

I don't know how/if you can do this with GRAM2, you saying 'gatekeeper' makes
me think you are talking about pre-webservices GRAM?
