I just had the problem again.  ALT-E did not work.  Sometimes it appears as 
though I have two instances of outlook running at the same time.
I can go to one and hit ALT-F4 and there is still one running.
What happened now is that when I hit the ALT-F4 the outlook did not respond.  I 
did try the ALT-E first.
