Nope, you can upgrade from home premium to ultimate, just not the other way
around.

For one, you will have to do a clean install. As far as I know there is 
no upgrade path from Vista Home Premium to 7 Ultimate, since they are 
two different editions, rather than two comparable ones. For two, yes 
you can use Narrator to finish setting up Windows, so long as it saw 
your sound card. If not, you will need either sighted help or a USB 
sound card.
