Hi:
One way to hear the title of the web page is to press control-shift-t.
 If you want to hear the web address of the page that you are on,
press f6 to get to the address bar and then shift-f6 to get back to
the body of the webpage.
I can't really answer your second question unless I know the address
of the page on which you are experiencing this phenomenon.
