In addition to Orlando's great explanation, it also allows you to have 
dictionaries based on the synthesizer or the language you're using (so if 
Eloquence mispronounces some word you can correct it, without adversely 
effecting Dec-Talk).

It's a large and complex app; like almost any such item, it does take some time 
and effort to learn what it does and how to use it.  It also benefits greatly 
from:
* reports to the author when you find something where you'd like it to behave 
differently, or have found a problem.
* and since it's free, some thanks to the author in public places (here on the 
lists; in the ratings and the comments of appGet and App Central, etc.), you 
get the idea.

(actually, I may speak too freely when I say it's "free"; David may say 
somewhere that he'd appreciate donations).

Its main purpose is pronunciation dictionaries that can handle all 
characters rather than mere letters. I use it to great effect with 
foreign words by entering phonemes for accented letter combinations 
that then pronounce longer words more correctly than with the normal 
Window-eyes exception dictionary architecture. There is also an 
applet in the package called Captuer Spoken Phrases, which allows the 
end user to change how Window-eyes interprets phrases and symbols 
within applications. For instance, if you wanted Window-eyes to say 
Application Key rather than Context Menu, you could not accomplish 
this through any native Window-eyes key label, color, graphic or
