When I worked on a drama/documentary on Victoria and Albert, we had
one of the Queen's equerries on set, giving advice on various
protocol issues. He showed me that when a man kisses a lady's hand,
he does not kiss the hand, but his own thumb! If you get the angle
right this is very simple and looks as though you are kissing the
hand. I don't know if this was 18th century practice, but was
obviously 19th century and up to modern times.
