I'd approach it by making a long strip of fabric, then put braid on it.  Cut
that into strips and "weave" it into a rectangle.  Place your pattern piece
on it at a 45 degree angle to the "weave.  I'd do a mockup first to see how
much height you need to get the puff.  I think you can see how the weave is
pushed together at the cuff to give it volume.

Thanks for your help with my pattern questions. I have decided to make a
loose gown out of my mid-green cotton velvet and an open gown out of a
brocade. I'm trying to use up my cupboard of fabrics. I would like to copy a
sleeve at http://www.earthlydelights.com.au/sleeve.jpg  for the loose gown.

Any ideas how this one might have been constructed?
