I don't know whether I understood correctly, maybe I'm completely out of the point, but here is what I use for taking measurements. There are all measurements I usually need except for these rarely taken (wrist c., crotch depth,...). It has abbreviations used in Czech, so I wrote in the fields where the measurements should be written down the approximate translation. This form isn't really my idea, but it's taken from the Rundschau patterndrafting magazine with some alternations. It's a good help for me, no need to worry that you forget to take some measurement. There are also blank fields for you to fill in some extra measurements and a space on the right side to make drawings etc.

I know what they want and what my budget is going to be - auditions for the show are next weekend, so I don't know who I have to costume yet. I am going to be at the first read thru and would like to get measurements, sizes, etc. then if at all possible. Does anyone have a good form they use for that kind of thing? I have an idea but I'd like to look at other people's suggestions. Thanks in advance.
