I don't know whether I understood correctly, maybe I'm completely out of the 
point, but here is what I use for taking measurements. There are all 
measurements I usually need except for these rarely taken (wrist c., crotch 
depth,...). It has abbreviations used in Czech, so I wrote in the fields where 
the measurements should be written down  the approximate translation. 
This form isn't really my idea, but it's taken from the Rundschau 
patterndrafting magazine with some alternations. It's a good help for me, no 
need to worry that you forget to take some measurement. There are also blank 
fields for you to fill in some extra measurements and a space on the right side 
to make drawings etc.
