The form I use most often is taken from The Costume Technician's Handbook.
It has a separate form for male and female with a sketch to show you where
the measurements are taken. Depending on the show, you may not need all the
measurements. 

Start at the top--

Head (around and over the top ear to ear)
Throat
Neck
Shoulder to shoulder , front / back
Armseye to armseye - f/b
Circumference of armseye
Bicep
Forearm
Wrist
Chest
Midriff or underbust for female
Waist (at navel for males)
Hip bone and waist to hip bone for women
Hips  and waist to hip
Thigh 
Above knee
Below knee
Calf
Ankle
Instep girth
Arch
Inseam --to below knee, to ankle, to floor
Waist to below knee
Waist to floor/ankle
Neck to floor - frnt/bck/side
Crotch -stride, girth, depth
One shoulder length
Shoulder to elbow
Elbow to wrist
Shoulder to wrist (not bent)
Armpit to wrist
Armpit to waist
Mid-shoulder to waist
Mid shoulder to chest level 
Top of shoulder to waist front/back
Neck to waist front/back
Bust tip to tip 
Across bust side to side
Trace the foot on the back of the sheet
Fill in the blanks for:
Name, character, phone, email, date, height, weight, shoe, shirt (men), bra
(women), tights, leotard, hand, hair color/length, etc.

VERY IMPORTANT!!! Allergies!!!

Space to note things like one shoulder obviously higher/lower; odd body
shape (these notes are often my notes that I don't really share with the
actor if I feel it might embarrass them.

I try to remember to ask things like "Are you pregnant? Are you seriously
losing/gaining weight? Lifting weights?"

Remember that the 12 year old is going to grow between measurements time and
opening night.

I have been hired to do costumes one of a local community theatre's summer
productions, and I've already talked to the director of the show and the
executive director of the theatre.  I know what they want and what my budget
is going to be - auditions for the show are next weekend, so I don't know
who I have to costume yet.  I am going to be at the first read thru and
would like to get measurements, sizes, etc. then if at all possible.  Does
anyone have a good form they use for that kind of thing?  I have an idea but
I'd like to look at other people's suggestions.  Thanks in advance.
