In most cases, yes patients will be returning for this and other episodes. 
CLinic nurses are also tasked with follow-up.  Presently, we are able to
associate the majority of patients with previous visits through dialog at time
of admission.  I think that this is mostly done by matching the initias/alias
DOB and sometimes address.

One strategy that was proposed was to use STD as the last name.  The name
string would then be STD,initials (ex STD,MPZ); or repeating the initlas twice
(ex MPZ,MPZ).  What are thoughts on these strategies?

In our present system, individual patients are asociated with a case number. 
For this to also be used as as an identifier, would it be feasible to introduce
a case number as a new data item to the patient file definition?
