CMS, and every commercial payor, already has software to do that, which 
IS used to determine payments. It's deeply flawed, for example, CPT 
31575, examination of the larynx is not payable with ICD-9-CM 478.32, 
vocal cord paralysis. How, do you know that the vocal cord is paralyzed 
unless you examine it?
