Another voice: Subject indexes are really useful if you want to include metadata and creative umbrella groupings, which may or may not be included as separate terms for keyword searching, as well as for any See references. My daughter's indexer was $3/indexable page, with 2 complimentary hours of edits, plus $35/hour after that for additional edits. Another indexer she spoke to asked for a flat rate of $1250-1500.
