Minor corrections

On p100 of the report about half way down, the comment preceding the defn
of (!!) talks about "Array index". Shouldn't this be "List index"? Array
indexing is elsewhere (!).

On p101 the large comment at the top gives certain rules obeyed by fold
and scan fns. All of these except the last use == to state the rule. The
last uses =.
