| On p100 of the report about half way down, the comment preceding the defn
| of (!!) talks about "Array index". Shouldn't this be "List index"? Array
| indexing is elsewhere (!).

| On p101 the large comment at the top gives certain rules obeyed by fold
| and scan fns. All of these except the last use == to state the rule. The
| last uses =.
