The CIFTI indices don't refer to a different surface, but they do exclude the vertices that fall inside the medial wall, for a modest reduction in file size. Similarly, voxels that are not inside a subcortical gray matter structure are also not represented in the file, which is a more significant reduction in file size.
You can use the -file-information command (possibly with -no-map-info if you aren't interested in the data values at the time) to get a quick summary of how many vertices and voxels are used in the various structures. You can also use -cifti-export-dense-mapping to get a text file for any structure that gives you the details of what each CIFTI index represents.
