Thanks for the review Haohui!
Good catch on the "using namespace" in the header, that must have ended up
there while I was splitting the files apart. I'll fix that, the comment issue,
and go back to using shared_ptrs to the promises to fix the race condition
similar to your fix on the old github branch.
