I have tried
   @math{b_l \ldots b_h}
   as you recommend but then I get in the dvi file
   b_l\ldotsb_h
   in italic.
   Any idea?

Please always tell us the version of Texinfo you are using.  For me

   @math{b_l \ldots b_h}

works fine and follows the documentation exactly, for both makeinfo
and texi2dvi.  I just tried it.

I used:

    $ makeinfo --version
    makeinfo (GNU texinfo) 4.3

    $ texi2dvi --version
    texi2dvi (GNU Texinfo 4.3) 1.10

By the way, please complain to the C++ author who first wrote and
adopted `::' years after it became a standard for Info.  He broke
backwards compatibility.
