I was having this problem until tonight when I was given the last piece of
the puzzle. My solution is as follows:

Use hlds_updatetool.exe (google it) to get a dedicated server outside the
steam client and run that for the server as you would any normal server.

In the steam client, right click on the game name and in the launch options
set a different port for the game.

Then start the game from the steam client press the toggle console key and
use the connect command from the console to connect to your server by its
real IP (not localhost or 127.0.0.1)
