first I would use cl_ent_rbox (or r_drawrenderbounds 1) to visualize the render 
box for the model.  Maybe it's really small or something?  Could be a problem 
with the entity you're using.

It's hard to debug without more information, but it seems like the most likely 
issue is with culling.  If setting r_portaltestents 0 fixes the problem, then 
you've got some kind of areaportal problem in the map.

It could be a lot of other things, but those are my first two guesses anyway.
