Do you run any cmdline parameters other than -tickrate, +maxplayers, etc? Are you running bots at all? If so, have you tried disabling bots? Have you tried removing all files relating to Mani-admin (including config dirs, etc) and trying again? Have you tried running HLDSUpdateTool and creating a fresh default SRCDS file structure?
I suggest creating a brand new copy of the source content from scratch, using the HLDSUpdateTool, and see if that exhibits the same problems.
