Say I want to ban BONERPHONE via console permanently (and kick him.)

Syntax is [Usage:  banid < minutes > < userid | uniqueid > { kick }]

rcon banid 0 979 kick

Unless they reconnect this ID will stay the same, and it's pretty easy to
get their ID (plus their connection time) if you know what you're looking
for.

Or, you can just limit the amount of name changes...or use a plugin to temp
ban after X amount of name changes.
