Point is, you can't depend on banning by name.  The best way to do this for
now is to do it by steam_id.

If the plans are still as they were in December of last year, the older
games will be ported to the orange box engine.  Once they do, I believe the
names will be locked down like they are in TF2.  Until then, you'll have to
live with the hassle.  

IIRC, isn't there a plugin that will kick for more than X number of name
changes?

I do, bro.  But when you're trying to spec someone in game and catch a 
cheater and their name keeps changing it makes it more difficult to nail 
them down so you can ban them.
