We use www.dot5hosting.com if you are just going to host files they have a great price. With them for almost a year now and no big issues. Something like $4.95 a month I think

Ive found that generally a server running lots of custom maps has a tolerance time of roughly 5 - 7 minutes for the average player before they will disconnect and not bother downloading. Another thing that I think would help immensely would be to list the speed and percentage of the download complete, the bars really aren't enough of an indicator, though this would be something valve would have to patch in.
