Though it may not apply to you, we had good success with the 64-bit hacked Apache on our Server 2008 x64 box. It made quite a difference. I am not claiming that the 64-bit was "better" but that the hacked version sure did run "happier" on the box than the stock Apache.
Yes, we check the useragent and referrer strings on .htaccess. Sure, somebody could still cheat it with their web browser, but directory browsing is still disallowed. I suppose "somebody" may have written a "leech-a-fastdl" addon option, but the logs don't show leech activity after applying the filters. For us, it was "good enough".
