I have this problem with my servers where when players leave, it still shows
them in the server in both HLSW and on the player count of the server.

People cannot join the server because the server thinks its full, when in
fact its empty.

Any help much appreciated

There is a SM plugin that gets this info for L4D, I'm sure it's not much
different for L4D2. No idea if that plugin still works.

in the console (not via rcon)

and then the console spammed with name (No Steam Logon) for every player on
the server?

and then end up with an empty server?

It has happened to me several times with the last update, not before it.

well im using a .bat file for server restarting, so it doesn't check the
status of the game server (like if it is frozen) only checks the proccess,
it becomes toatally unresponsive. tools like Server Checker auto restart the
server when this happens (as to everyone it seems offline).

We had a few fishy l4d2 games yesterday were after the first map the entire
server got kicked for no connection to steam.

I'm finding that some custom maps/campaigns aren't working correctly when
uploaded to my server. (these work fine locally with Addon Support
installed)

I reckon we need a method of installing the pak02**vpk files to the server.

Can HLSupdateTool do this? If so, how?

I'm trying to set up valve rotation server, everything is good but the
"waiting for players" time is too long. Is there any cvar that makes it
shorter?

I do not want to disable mp_enableroundwaittime but I feel like 28 seconds
is too long.
Also I've seen servers where waiting time starts from 20 seconds or so but
GOES AWAY
as most players join Red/Blue.
Is this possible, or am I mistaken?

The cvar is mp_waitingforplayers_time it happens to be a hidden cvar. You
can use SourceMod to set it using sm_cvar mp_waitingforplayers_time.

at the time on one of them i was actually on the server and yes it told me i
had no Steam Logon, but i was clearly logged in...

There's also:
"mp_waitingforplayers_restart" = "0"
  game
  - Set to 1 to start or restart the WaitingForPlayers period.
"mp_waitingforplayers_cancel" = "0"
  game
  - Set to 1 to end the WaitingForPlayers period.

So you could write a simple addon that cancels the waitingforplayers 
time if x players join teams
