The issue appears to be the new tf_mm_servermode cvar.

The default value of 0 overwrites sv_region and sets it 255.

See below,

Setting 'sv_region 255 ' due to tf_mm_servermode

"tf_mm_servermode" = "0"
 game
 - Activates / deactivates Lobby-based hosting mode.
   0 = not active
   1 = Put in quickplay pool (based on current map)
