I ran dual monitors a couple of years ago and had the same issue.

At the time I did a bit of research and found the problem was that the
OpenGL specification does not make any provision for dual monitors; so when
you try to use a full-screen OpenGL application, it doesn't know what
monitor to run on and crashes. (At least that's my layman's understanding,
as somebody who knows next to nothing about OpenGL.)

I think the more recent Detonator drivers have resolved this problem
(perhaps via the TwinView stuff), if you're using Nvidia cards.

Oh, when I was doing this I was using two video cards (1 AGP and 1 PCI) as
there weren't any dual-head cards readily available. But that probably
doesn't make much difference in itself.
