People have reported some success with making the custom files readonly (I think 
Alfred may have said it too).
How good or viable a solution this is in your case is up to you to decide.

I have a custom hlds_run file which checks that the process isn't being
run in root and will restart the game process even if somebody does
'rcon quit'.

Does anybody know of a way, or are Valve willing to add; a method of
telling Steam to skip updating this file?  Ideally a plain text file
with a list of filenames to skip when updating.

The same feature would be quite nice in the client too, but that's not
for this list.
