I had my hlds_run owned by root user with read & execute allowed by
steam but when it came to running an update it bails saying it cannot
access it.

Obviously it can read it, just not write to it.

How good or viable a solution this is in your case is up to you to decide.

I have a custom hlds_run file which checks that the process isn't being run in root and will restart the game process even if somebody does 'rcon quit'.

Does anybody know of a way, or are Valve willing to add; a method of
telling Steam to skip updating this file? Ideally a plain text file
with a list of filenames to skip when updating.

The same feature would be quite nice in the client too, but that's not
for this list.

Regards,

Alastair.
