I have seen this with CSS for about 1 year now. Normally off by 1, but I have seen it off by 2 and 3. I think this is reset if I restart the server. Also, server info shows 1 or 2 player names, even with no one connected. I can even restart the server and the players are still listed. So, this seems to be a client issue.
