You need the host port open for each server you run. So that's probably 27015,6,7...27014+number_of_forked_servers_you_started, or whatever you set this to. The other ports you specified are either outgoing only (if your firewall allows all established/related connections no action is required for this), or things like hltv (which I noticed you disabled), or things like the netconport console (which I assume you don't want external access to open)
