In relation to the accounts, I noticed that the working account is a STEAM_0:0: account while the one where items aren't working is a STEAM_0:1: account. As I recall, this related to which Steam Auth server they use.

Long story short: I have two Steam accounts.  At the present time, my
older account (from 2003, I've had TF2 since 2008) can access the item
server and do inventory management with no problems.

My newer account (from 2011) can not access the item server.

This is from the same PC, so I assume there is some sort of
infrastructure error going on.
