(Not actually) Quoting John Stacy: What Molly White said. Get yourself evaluated by a professor at a top medical school or teaching hospital, preferably more than one. IIRC, you're in the middle of nowhere. This is definitely the time to pony up the bucks to travel to where such doctors are.
I know from personal experience, the details of which need not detain us here, that professors at medical schools and teaching hospitals can work wonders. Not only do they know the latest research and treatments, but, IMHO even more important, they have the nerve to try things that will probably work, but that local specialists won't attempt.
