I have actually yet to encounter someone who was great at both designing and programming. But it has to do with my opinion of what a great programmer is. I have encountered plenty of people who do both and while in my opinion they were great designers their programming skills were less than desirable. Of course those people though did get the job done and typically based upon their completed work were the most desirable people to get for projects. My list just includes various databases and programming languages, I once would include Gimp on there ;)
