Is the banding mostly in lighter or darker region of the image? Unfortunately the Print Quality Diagnostic (PQD) page does not always reveals which printhead may be causing the problem. If you have access to a RIP to can print CMYK gradients and pinpoint exactly which printhead needs to be replaced. Perhaps a combination of both PQD and printhead alignment page may help you determine the root cause.
