I have used all types of cartridges including after market ones. We even
have a continuous ink system we are testing now. There doesn't seem to
be any significant pattern to the failed printheads only that it almost
never happens with black.

It's possible you have a bad cartridge, or air in the line that feeds 
the head that consistently fails. I went through lots of heads until I 
found this out.

You didn't mention the pattern of head failures in your machines. Also 
whether or not you use after market cartridges.
