I found some better settings: Markup-margin size=0pt instead of 9pt, no odd white holes in the middle anymore :) layout x0=7.5pt instead of 6.5pt, leaves a small margin at the left and bottom side, barely noticeable. 7.4pt and a little more y0 probably does the trick. Hole size seems fine, but not for my (silver Plextor) cd's (they have a little piece of uncovered area, printer goes to the thick circle. I hope this is useful.
