Are you certain that your start_url attribute is set correctly in the config file? With the default configuration, ht://Dig should not care whether the extension is .htm or .html. If you feel that both ht://Dig and your web server are configured correctly, you might try running htdig with -vv or -vvv and examining the output to determine why some pages are not being indexed.
