It's just another search. The term "refine" is perhaps a bit misleading. You can refine your search by adding words to what's already there, but you can also start fresh with a completely new set of search terms. The follow-up search forms are defined in the template files (e.g. header.html, nomatch.html, etc.), and there's not a whole lot that's special about them other than the fact they fill in some values using template variables, to propagate input parameters from the earlier search into the new form.
