There are a few reasons this can happen and you haven't really given 
enough information to narrow down the cause. For example, in addition to 
documents that have been indexed, ht://Dig keeps "stubs" for document 
URLs that it has discovered but not indexed yet. Since text of links 
pointing towards documents is included as search terms, these can come 
up in results.

If you're using 3.1.x these "stubs" are removed by htmerge. If you're 
using a 3.2 beta, these are not removed by htpurge by default--if you 
don't want them, you should set the "remove_unretrieved_urls" attribute.

Beyond this reason (IMHO the most likely), you may have a document that 
has a title tag, but because of malformed HTML, htdig doesn't find it 
when indexing.
