Sorry, but I am also at a loss. If you are sure the databases are being built from scratch and that there isn't a problem with limit_urls_to, then it sounds like a bug of some sort. However I have never seen this happen myself.
As a work-around, you might try using the exclude_urls attribute (http://www.htdig.org/attrs.html#exclude_urls).
